It’s a whole new act so you can’t go back to driftwood again. Be sure to complete all the quest you care about before leaving.

Killed Garvan Go back to Garvan but don’t attack him inside the tavern! Instead feed him Tainted Stew, it can be crafted from Meaty Stew and any Void-Tainted Fish. He will then move to the outhouse behind the tavern. Follow him, confront him then kill him.

Death fog lingers in the air. So what you want to do is to create steam. My way was to use a water arrow and follow with a fire arrow. This removed the death fog, ever so slowly.

The platform is clear of Deathfog. Teleport to that platform somehow, give one pyramid to another party member, unchain, and then teleport your unchained hero who should also have a pyramid to the shore of the island. Switch to your chained party and activate the appropriate pyramid. Voila!

To keep Arhu alive, you may want to teleport him to a safe corner, and avoid crowding around him so that Lord Kemm won’t use his whirlwind attack on your party and hit Arhu as well when he appears. If he dies, you can always use Spirit Vision and still speak to him, though.

There is deathfog on the ground inside of a room which you cannot take it out of, there is deathfog around Bloodmoon Island which you can terrain transmutation all through out the map, and there is some lava near Hannag that you can terrain transmutation all through out the map.
