Care and support planning is a defined process which helps people set their own aims, and then secures the support and care that are needed to achieve them. It is the key that unlocks person centred, coordinated care.

Can you write your own care plan?
You can: • make the plan yourself. You may like your family, friends, Social Worker or Care Manager to help you with ideas and identify options. They can give you as much or as little help as you need to write your support plan. They can share examples of other plans.
