Yes it does scale and it’s much tougher.
Can you keep playing after beating bloodborne?
Yes and no. There are three potential “final bosses” to Bloodborne, and Gherman is one of them. So, after defeating Gherman, if you’ve failed to meet the requirements for the hidden boss (details at the bottom to avoid spoilers) the game will enter a cutscene that will end your players New Game cycle.
Is bloodborne NG+ harder?
NG+ isn’t much harder, no. If you’re finding end game on base game easy enough, NG+ will be a breeze. Enemies hit a little harder but at this stage you’re probably perfectly capable of avoiding getting hit by most things, and if you’ve invested into vit, that won’t matter anyway.

Yes and the difficulty in the chalice dungeons are not affected by NG+. If you want to level up to complete the chalice dungeons you’d be better off doing it in NG+ because you get a lot more blood echoes from enemies.
